Admin can now activate or deactivate a coordinator from the Commission screen.
Each coordinator card shows its status and downline size.
Deactivating also deactivates that coordinators sub-coordinators and all their tellers, so they can no longer log in.
Reactivating turns the whole downline back on.
A confirmation dialog shows how many accounts will be affected before anything changes. No data is deleted.

Game Settings now has two fields per game: Coordinator Win and Teller Win.
Teller sees/pays the teller rate; coordinator and admin see the coordinator rate.
